CLINICAL EVALUATION OF INTRAVENOUS DILTIAZEM IN PATIENTS WITH UNSTABLE ANGINA

Objective To investigate the clinical effect of intravenous diltiazem on patients with unstable angina(UA) . (Methods Diltiazem) was administered intravenously to 76 UA patients who had poor response to nitrates. An intravenous injection of the first dosage of 0.25 mg/kg diltiazem was given to those while angina was attacking on admission and continued with intravenous drip; those without angina attack on admission, an intravenous drip only was given. The initial dosage was 1 μgkg~(-1)min~(-1), if angina remained, the dosage was added to 3 μgkg~(-1)min~(-1) or 5 μgkg~(-1)min~(-1). If the attack was controlled, the drug should last for 48 hours with the effective dosage. Results Among the 76 patients, angina attack was controlled in 38, 22 and 10 patients at dosage of 1 μgkg~(-1)min~(-1), 3 μgkg~(-1)min~(-1) and 5 μgkg~(-1)min~(-1), respectively. The total response rate was 92%. No severe side effects were observed. Conclusion The intravenous use of diltiazem is safe and effective for UA patients.
