Relation between circadian blood pressure profile and carotid artery damage

Objective The aim of this study is to determine the relationship between carotid artery damage and circadian blood pressure changes. Methods A total of 595 hypertensive patients underwent 24h non-invasive ambulatory blood pressure monitoring, and carotid intima-media thickness as well as the occurrence of plaque were evaluated by ultrasonography. Results The nocturnal dipping (decrease of blood pressure 10% during night time) and non-dipping (decrease of blood pressure ≤10% during night time ) profile was found in 180 and 415 patients respectively. The mean intima-media thickness (IMT) of the left and right common carotid artery and the maximum IMT of common carotid artery were significantly greater in non-dippers as compared with dippers (P 0.05). And a significantly higher prevalence of plaque was observed in non-dippers as compared with dippers (P0.05). Conclusions Hypertensive patients with non-dipper pattern showe a significantly greater extent of carotid structural alterations comparing with patients with dipper pattern. The circadian rhythm of blood pressure seems to represent an important factor of carotid wall damage.
