RELATION OF CHOLIC ACID AND BILIRUBIN TO BILE PIGMENT PRECIPITATION A RESEARCH ON PIGMENTGALLSTONE FORMATION

In 1973, authors found that the precipitates of the bile from the patients with pig-ment stones was nearly 3 times than that of the patients with other diseases. Subsequent re-search disclosed that dry-powder like precipitates were identical in appearance, solubility andinfra--red absorption spectrum with pigment stones, and contained calcium, polysaccharides,proteins, glycoproteins, cholesterol and monomer and polymer of bilirubin. Usually, the bile specimens produced quite a lot of such pigment sediments with highconcentration of bilirubin, during 3 to 5 days after biliary operations, but decreased or dis-appeared gradually several days later, except in case of muddy stones. The amount of thesediments also increased if high fever occured after cholangiographic examination. The con-centration of the bilirubin in the bile was in proportion to the amount of pigment sedi-ments. However, the high concentration of bilirubin was often associated with the decrease ofcholic acid content, which was lowed during the initial stage after operation and it increasedlater, though the bile appeared light in color and thin in appearance. yet the concentrationof cholic acid was of high level, and sediment was lesser; conversely, when the bile ap-peared dark and thick, the concentration of cholic acid was lower and the sediment wasgreater. When dehydrocolic acid or some traditional Chinese medicine were taken or ox bilesalts were added into the bile before incubation the amount of pigmented sediments de-creased. The authors also found the concentration of the canine cholic acid and its cholic acid/bilirubin ratio far exceed that of human being, and had caused the dissolving and disinteg-ration of the human pigment stone. In conclusion, authors consider that long-term existance of the reduction of bile acid/bilirubin ratio might be a major factor of pigment stone formation. Some pathological con-dition, such as infection or obstruction of the biliary system may damage the hepatic cellsand cause this abnormal bile metabolism. It also explains why cholesterol crystal were fre-quently dispersed among the pigment granules in the pigment stones or muds. It might be interested to note that authors finding is somewhat different from that ofMaki.
