Effects of the different induction anesthetic drugs on cerebral oxygen metabolism at brain tumor operations

Objective To study the effects of propofol and thiopental sodium on cerebral oxygen metabolism during brain tumor operations by placing retrogradely a jugular catheter to monitor jugular bulb oxygen saturation. Methods25 patients of brain tumor undergoing neurosurgery were randomly divided into 2 groups: group I with 12 patients, and group II with 13 patients. During induction, group I was administered with midazolan 0.06 mg/kg, fentanyle 2 μg/kg, propofol 2 mg/kg and vencuronium 0.15 mg/kg, and only propofol was replaced by 2.5 % thiopental sodium 5mg/kg in group II. Radial artery and jugular vein blood gases of the anesthetic drugs were monitored simultaneously at 5 min before inductive application (T1), and 5 min (T2), 30 min (T3) after the induction, and then SjO 2 ?CEO 2 (CEO 2 =SaO 2 -SjO 2 )and Da-jO 2 were calculated. Results There was no significant difference between the two groups in SjO 2 and Da-jO 2 before induction. However, SjO 2 and Da-jO 2 at group II were significantly higher than those at group I at 5 min and 30 min after the induction (P0.01), but CEO 2 at thiopental sodium group II was lower than that at group I (P0.05). Conclusion Propofol is more inductive in influencing cerebral metabolism than thiopental sodium. It exerts its mechanism in that it may lower blood pressure so as to decrease cerebral perfusion and then work on CMRO 2 /CBF.
