Clinical study on the treatment of inflammatory nipple discharge with local drug perfusion

Objective To observe the intraductal structure and lesion nature of the nipple discharge under mammary ductoscope and to explore a new therapeutic method of inflammatory nipple discharge.Methods Of 200 cases with nipple discharge examined by mammary ductoscope,40 cases diagnosed as mammary duct ectasia or inflammation (3 cases of mammary duct ectasia and 37 cases of mammary duct ectasia complicated with inflammation ) were selected and treated with dexamethasone and gentamycin for 3 to 4 times (perfused every 2 days as one times )in which 2 cases were perfused for 8 times.Results All the patients were followed up for 3 to 6 months,36 cases got healed and 4 cases alleviated ( 1 case of mammary duct ectasia and 3 cases of mammary duct inflammation ).Two cases with disappearance of nipple discharge after 2 therapeutic courses were reexamined with mammary ductoscope 2 months later: one case was found with less inflammation and exudation,smooth surface and disappearance of congestion; and the other case still with relatively large amount of gray flocculus and rough ductal surface. Conclusion Treating inflammatory nipple discharge with local drug perfusion is clinically effective with simple,atraumatic and uncomplicated manipulation.It can prevent tissue trauma and mental burden of the patient caused by operative intervention,providing a new therapeutic measure in clinic.
