Application of platysma myofascial flap in reconstruction of larynx

Seventy-two patients with laryngeal carcinoma were treated with laryngectomy and reconstruction using platysma myofascial flap from 1988 to 1994.There were 66 males and 6 females,ranging in age from 35 to 70 years.Clinical types of these patients were supglottic cancer in 15 (T_1N_0M_02,T_2N_0M_05,T_2N_1M_02,T_3N_0M_04,T_3N_1M_02),glottic cancer in 55(T_1N_0M_08,T_2N_0M_034,T_2N_1M_01,T_3N_0M_0 12),and subglottic cancer in 2 (T_2N_0M_0 1,T_3N_0M_0 1).According to the clinical type and T stages,51 patient were surgically treated with partial laryngectomy,and 21 with near-total laryngectomy.The platysma myofascial flap was used to reconstruct the laryngeal defect in all patients. Sixty-nine patients were followed up over 3 years and three patients were lost.Results showed that 3-year survival rate was 84.7%(61 in 72),decannulation rate was 91.7% (66 in 72).Eight patients died because of recurrence of cancer.The indications of surgery and technique of reconstruction were discussed.
