A Meta-analysis of treatment of sudden deafness with intratympanic glucocorticoids administration

OBJECTIVE To evaluate the efficacy and safety of intratympanic glucocorticoids(IT-G)in the treatment of sudden deafness.METHODS The quality of selected randomized controlled trails that treated sudden deafness with IT-G was evaluated and Metaanalysis was conducted.RESULTS 1.The effectiveness of IT-G in patients with sudden deafness who showed poor response to conventional therapy:5 studies were selected,pooling results of Meta-analysis indicated that IT-G was statistically significantly superior to placebo in the treatment of sudden deafness as salvage treatment(OR=12.41,95%CI=4.32~35.64,P0.00001);2.The effectiveness of combination therapy(IT-G+systemic glucocorticoids administration)as 1st-line treatment in patients with sudden deafness:2 studies were selected,pooling results of Meta-analysis showed no difference between the two groups(OR=3.12,95%CI=0.35~27.60,P=0.31);3.The effectiveness of IT-G as 1st-line treatment in patients with sudden deafness:2 studies were selected,pooling results of Meta-analysis indicated that there was significant difference between the two groups(OR=2.57,95%CI=1.14~5.83,P=0.02);5 studies evaluated the safety of IT-G,and no serious adverse reaction was recorded.CONCLUSION The clinical evidence available so far indicated that IT-G administration after failed conventional therapy is a safe and effective treatment for sudden deafness,IT-G as 1st-line treatment is more effective than systemic glucocorticoids administration,and there is no evidence to support that combination therapy as 1st-line treatment is more effective than systematic glucocorticoids administration alone for sudden deafness,although rigorously designed large sample size randomized double blind controlled trials are required to further demonstrate and support the conclusions.
