Prevention and treatment of pulmonary embolism following major surgery of gastrointestinal malignant tumors

Objective:To study the etiology,diagnosis,treatment and precaution of pulmonary embolism following major surgery of gastrointestinal malignant tumors.Methods:Thirty-five cases of pulmonary embolism following major surgery of gastrointestinal malignant tumors admitted to our hospital between June 2001 and January 2010 were analyzed retrospectively.Results:D-dimer checking was performed on the 35 patients,and the positive rate was 100 %.Spiral CT pulmonary angiography was performed on 32 patientsand abnormal findings were found in 30 cases.Twenty-four patients were cured through the treatment of oxygen therapy and anticoagulation.Six patients recovered through the treatment of assisted respiration by respirator,thrombolysis and anticoagulation.Four patients died of respiratory failure,and 1 patient died of wound infection and sepsis.Conclusion:Gastrointestinal cancer and surgical treatment are the risk factors of pulmonary embolism.The examination of pulmonary arteriography by spiral computed tomography in combination with D-dimer is helpful to the rapid diagnosis of pulmonary embolism.It can mitigate the pulmonary vascular blockage and degrade the fatality by giving patients the treatment of thrombolysis and anticoagulation.
