Preoperative assessment of liver functional reserve by the retention rate of indocyanine green at fifteen minutes and effective hepatic blood flow

Objective This prospective study was performed to examine the value of retention rate of indocyanine green at fifteen minutes(ICGR15) and effective hepatic blood flow(EHBF) in preoperative liver function assessment.Methods ICGR15 and EHBF were measured non-invasively and instantaneously using pulse dye densitometry(PDD) method before surgery in 76 patients who underwent anatomical liver resection for hepatocellular carcinoma(HCC).The relations of ICGR15,EHBF,Child-Pugh class and postoperative complications were studied.Results The ICGR15 was lower in patients with Child class A than in Child class B,and the results of EHBF were just on the contrary;The IC-GR15 was 18.4±5.6% in patients with postoperative complications and 8.1±4.0% in patients without(P0.001);The EHBF was 0.833±0.157 L/min in patients with postoperative complications and 1.154±0.196 L/min in patients without(P0.001).Conclusions ICGR15 and EHBF are good predictive indicators for predicting postoperative complications.Combining Child-Pugh classification and ICGR15 can evaluate liver functional reserve more precisely.
