Focal Cleaning and Xenogeneous Bone Graft with Autologous Bone Marrow Cells in the Treatment of Talus Osteonecrosis at Early Stages

Objective To investigate the minimal invasive method in the treatment of talus osteonecrosis at earlty stages.Methods From Jun 2000 to Sep 2002, 17 patients (17 taluses) with talus osteonecrosis at early stages were treated with focal cleaning and xenogeneous bone graft with autologous bone marrow cells.According to Ficat and Arlet classification system, 14 taluses were stage Ⅱ, 3 were stage Ⅲ. Patients were followed up from 24 to 51 monthes. The outcome was evaluated by the American Orthopaedic Foot and Ankle score (AOFAS), the progression of radiographic stages, and the need of additional treatment.Results The mean preoperative and postoperative AOFAS was 61.3 and 91.2 points respectively.Seventeen taluses were radiographical stable.One talus was later treated by ankle fusion.Conclusion The focal cleaning and xenogeneous bone graft with autologous bone marrow cells is effective in the patients with talus osteonecrosi at early stage. Its relative simple and minimal invasive.
