Replantation of Rotational Avulsion Injury of Complete Severed Wrist

Objective To study the effect of improved operation means about the replantation of rotational avulsion injury of complete severed wrist. Methods The new protocol was used in 9 of 11 patients with complete severed wrist.①Before the fixation of wrist joint,the thumb was fixed in the position of opposition and extension by Kirschner wire.②The important functional tendons were restored using tendon transfer firstly.Part of tendons were stitched by knot method.In contrast to the traditional means,more tendon were stitched in the new.③Superficial vein of the anterior aspect of the wrist and each of the vein radialis and vein ulnaris was anastomosed.The arteries were anastomosed early.④Carpal tunnel was cutted and part of it was excised. Results All of the 11 cases of replantation survived.After 12~36 months follow-up,the restoration of sensor function and motor function,4 cases are of excellent,5 good and 2 not bad,excellent and good rate 81.8%.Conclusion The advantages of the improved operation protocol about the replantation of rotational avulsion injury of complete severed wrist are decreased contractures of the first web space,good thumb opponensplasty and preventting tendon from adhesion etc..
