Microsurgerical treatment of hypertensive cerebral hemorrhage in basal nuclei zone by:analysis of 110 cases

Objective To investigate the treatment effects of hypertensive cerebral hemorrhage in basal nuclei zone by microsurgery. Methods 110 cases of hypertensive hemorrhage in basal nuclei zone were analysised retrospectivly. According to the volume of hematoma ,with or without cerebral hernia ,breaking into ventricle or not,the operative condition, different microsurgical approach were applied to these cases. The therapeutic results of different operative periods were compared. We also compared the effects with the cases after microinvasive operation simultaneously. Results Of all the cases, the mortality was 14.5%(16/110). Only 1.7% of cases without cerebral hernia or with hematoma volume less than 70ml treated by craniotomy with a small bony flap died. 29.4%(15/51) of these cases treated by large bony flap died. Among the large bony nap group, 22.5%(7/31) cases treated with decompression died. 40%(8/20) cases treated with evacuation of hematoma died. The mortality of microinvasive group was 30.4%(7/23).and 43.8% in total. Conclusions Microsurgery is more effective than the traditional craniotomy and microinvasive operation. The effects is better if the operative opportunity is ealier. All cases shouldn't be applied the same method. On the contray different methods should be applied according to different conditions.
