Value of procalcitonin to assessing the prognosis of elderly patients with septic shock

Objective To explore the dynamic changes of procalcitonin and the value to assessing septic shock in the elderly patients.Methods Ninety-seven elderly patients with septic shock were scored by Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ)and Sequential Organ Failure Assessment(SOFA)at admission.After 28-day follow-up survey,the patients were divided into death group(n=32)and survival group(n=65)according to the prognosis,and were compared the changes of procalcitonin,C-reactive protein(CRP),APACHE Ⅱ score and SOFA score between two groups,and the changes of procalcitonin on the first and 7th day to analyze the correlation of procalcitonin with SOFA score and APACHE Ⅱscore.Results There were no significant differences in the procalcitonin levels on the first day and 7th day between two groups(P0.05).The lactic acid level((6.18±3.52)mmol/L),SOFA score(11.26±4.39)and APACHE Ⅱ score(20.38±5.72)were significantly higher on the first day after admission in death group than those in survival group((4.42±3.47)mmol/L,6.87±3.54,17.65±4.79)(P0.05).There was no significant difference in CRP level at adminssion between death group((64.7±35.2)mg/L)and survival group((58.2±23.5)mg/L)(P0.05).The procalcitonin level on the 7th day was significantly higher in death group((10.82±3.58)μg/L)than that in survival group((1.25±0.73)μg/L)(P0.01),and had no significant difference in comparison with that on the first day((9.98±3.42)μg/L)in death group(P0.05).The procalcitonin level was significantly lower in survival group on the 7th day than that on the first day((10.12±4.56)μg/L)(P0.05).The procalcitonin level was positively correlated with SOFA score and APACHE Ⅱ score on the 7th day(r=0.625,P=0.001;r=0.646,P=0.002).The AUC of procalcitonin for predicting death was 0.831.When procalcitonin was 2.0μg/L,the sensitivity was82.4% and specificity was 71.6% for death.Conclusion Procalcitonin can be used to assess the prognosis of elderly patients with septic shock,and high-level procalcitonin indicates a poor prognosis.
