The Efficacy of oral Chinese Herbal Compound Prescriptions Supplementing Qiand Activating Blood Circulation in the Management of Urine Protein in Diabetic Nephropathy:a meta-analysis

Objective:To assess the efficacy of Chinese herbal compound prescriptions in the management of urine protein in diabetic nephropathy(DN).Methods:Search the following database:Medline(1978~2007),CBM disc(1978~2007),CNKI(1980~2007),VIP(1989~2007).Randomized controlled trials(RCTs)which compared Chinese herbal prescriptions with angiotensin-converting enzyme inhibitor(ACEI)/angiotensin Ⅱ receptor blocker(ARB)were included.Two independent reviewers evaluated the methodological quality of included studies and meta-analysis was performed.Results:72 Chinese RCTs were included in this review.All trials were classified as grade C except one trial which was grade B.The results of meta-analysis demonstrated that compared with ACEI alone,Chinese herbal prescriptions supplementing Qi and activating blood circulation could significantly decreased the 24-h Urinary albumin excretion rate(UAER)(WMD=-31.10;95%CI=-47.14,-15.07;P=0.000 1);when combined with ACEI,herbal compound prescriptions could significantly decreased UAER(WMD=-26.55;95%CI=-34.36,-18.74;P0.000 01),too.Compared with ACEI alone,herbal compound prescriptions could significantly decreased urinary albumin excretion(UAE)(WMD=-32.83;95%CI=-40.31,-25.35;P0.000 01);when combined with ACEI,the reduction was also significant(WMD=-56.29;95%CI=-100.83,-11.74;P=0.01).Herbal compounds could significantly decrease the 24-h urinary protein excretion(WMD=-0.30;95%CI=-0.45,-0.14;P=0.000 2),as compared with ACEI alone;when combined with ACEI,herbal compounds could also significantly reduced the 24-h urinary protein excretion(WMD=-0.63;95%CI=-0.87,-0.40;P0.000 01).The comparison between herbal prescriptions and ARB is similar to that of ACEI.Conclusion:The poor methodological quality of included studies seriously weakened the validity of the meta-analysis.The efficacy of oral Chinese herbal compound prescriptions supplementing Qi and activating blood circulation in the management of urine protein in diabetic nephropathy should be determined by more RCTs of high quality.
