Study of perfusion computed tomography imaging in early acute cerebral infarction

Objective To evaluate the feature of perfusion computed tomography imaging (PCTI) in early acute cerebral infarction (ACI).Methods Thirty-three patients of early ACI were examined by routine CT and PCTI,and all of them were re-examined after 48 hours.Cerebral blood flow(CBF),cerebral blood volume(CBV),time to peak(TP) in infarction core and ischemic penumbra,ischemic region and opposite corresponding region were analysed quantitatively and compared.Results In 33 cases 11 had routine CT positive results,and 29 had PCTI positive results.Except 1 case,CBF,CBV in ischemic region were lower and TP was longer than that in the opposite corresponding region.Furthermore,CBF((16.79)±(4.96)) ml·min~(-1)·100 g~(-1) of ischemic region was significantly lower than that of the opposite corresponding region ((28.11)±(6.97)) ml·min~(-1)·100 g~(-1)(P(0.01)),and CBV ((3.44)±(0.87)) ml/g was lower than the latter ((4.66)±(1.48)) ml/g (P(0.05)),TP((26.01)±(5.04)) s was longer than the latter ((22.31)±(5.36)) ml/g(P(0.05)).CBF((6.21)±(2.71)) ml·min~(-1)·100 g~(-1),CBV((1.40)±(0.86)) ml/g of infarction core were significantly lower than that of ischemic penumbra(P(0.01)),CBF((19.28)±(4.73)) ml·min~(-1)·100 g~(-1),CBV((3.94)±(1.22)) ml/g,TP((31.61)±(3.52)) s of infarction core was significantly longer than that of ischemic penumbra ((23.80)±(5.90)) s(P(0.01)).Conclusion PCTI may display ischemic region in the early ACI period,determine the position,area and degree of the ischemic lesions.It may find ischemic penumbra,analyse the blood perfusion status,and instruct the treatment.
