Expression and significance of eIF4E,C-myc and CDK4in gastric carcinoma and premalignant conditions

[Objective]To study the expression and significance of eukaryotic initiation factor 4 E(eIF4 E),C-myc and cyclin-dependent kinase 4(CDK4)in the diagnosis of gastric carcinoma and premalignant conditions.[Methods]A total of 202 gastric mucosal biopsies and surgical resection specimens were collected from March 2016 to December 2017 in Wuhan Puren Hospital,including 23 cases of chronic non-atrophic gastritis,30 cases of chronic atrophic gastritis,25 cases of gastric mucosal low-grade intraepithelial neoplasia,27 cases of gastric mucosal high-grade intraepithelial neoplasia and 97 cases of gastric carcinoma(including adjacent tissues of 2-3 cm from the edge of the carcinoma).The expression of eIF4 E,C-myc and CDK4 in the above specimens were detected by immunohistochemistry.[Results]There was no positive expression of eIF4 E protein in gastric mucosa of patients with chronic non-atrophic gastritis.In premalignant conditions of gastric carcinoma and gastric carcinoma,the expression of eIF4 E was significantly different,showing an increasing trend(P0.01).Furthermore,Lymph node metastasis group and advanced group were significantly higher than those without lymph node metastasis group and early group(P0.01).The positive expression rate of eIF4 E protein in gastric carcinoma paracancerous atrophic gastritis group,paracancerous low-grade intraepithelial neoplasia group and high-grade adjacent intraepithelial neoplasia group was significantly higher than that of non-cancerous corresponding group(P0.01 or P0.05).In addition,there was a significant positive correlation between the detection of eIF4 E protein in the adjacent to carcinoma and the corresponding carcinoma(r= 0.343,P0.01).There was no positive expression of C-myc and CDK4 in gastric mucosa of patients with chronic non-atrophic gastritis,and the positive expression rate increased gradually in premalignant conditions of gastric carcinoma and gastric carcinoma.The positive expression rates of C-myc and CDK4 in premalignant conditions of gastric carcinoma group and gastric carcinoma group were significantly higher than those in chronic non-atrophic gastritis group(P0.01).There was also a significant difference between the gastric carcinoma group and the premalignant conditions of gastric carcinoma group,the high-grade intraepithelial neoplasia group and the chronic atrophic gastritis group and the low-grade intraepithelial neoplasia group(P0.01 or P0.05).There was a very significant positive correlation between the detection of eIF4 E and C-myc and CDK4 in gastric carcinoma tissues,respectively(r= 0.652,0.494,P0.01),and there was also a significant positive correlation between the detection of eIF4 E and C-myc and CDK4 in premalignant conditions of gastric carcinoma tissues(r= 0.775,0.694,P0.01).[Conclusions]Tracing the expression of eIF4 E protein in premalignant conditions of gastric carcinoma may contribute to the early detection of gastric carcinoma.The increased expression of c-myc and CDK4 in gastric carcinoma and premalignant conditions may be related to the high expression of eIF4 E,which provides clues for the in-depth study of the pathogenesis of gastric carcinoma.
