A functional MRI study of language lateralization in left medial temporal lobe epilepsy

Objective To lateralize hemispheric language dominance in left medial temporal lobe epilepsy(MTLE) using fMRI. Methods Eight patients with left MTLE were evaluated. Auditory semantic judgment (AJ) paradigm was used. The fMRI data were collected on a 3T MR system and analyzed by AFNI (analysis of functional neuroimages). On the basis of the generated activation map,hemispheric dominance for language was calculated in the Broca's and Wernicke's regions using laterality indices (LI).Results The group analysis showed that the mean LI were -0.29 and -0.25 resprctively in left MTLE patients,which indicated right hemisphere dominance. The individual analysis showed that atypical language representation in Broca's region was found in 75% of left MTLE patients and right language representation in Wernicke's region was found in 62.5% of left MTLE patients.Conclusion fMRI is a valuable technique in lateralizing hemispheric language dominance in left MTLE.
