Observation on the effect of continuous intravenous diltiazem infusion in perioperative patients with severe hypertension

Objective: To evaluate the antihypertensive effect and safety of continuous intravenous diltiazem infusion in perioperative patiente with severe hypertension. Methods:Opened clinical trial. Twenty-three peri-operative noncardiac surgery patients with hypertension were studied. Intravenous diltiazem was administered firstly with infusion of 5~10 μg/(kg·min), or with micro-syringe, then gradually increase 5~10 μg/(kg·min) every 5 minutes, until the dosage up to 10~20 μg/(kg·min) within 30 minutes. The changes of SBP, DBP and heart rate were observed. Results:The blood pressure and heart rate were monitored and compared before and after 10, 60minutes of intravenous diltiazem infusion. The effect on blood pressure was significant, SBP/DBP was gradually dropped from {210.8}±{19.6}/{128.4}±{10.3}(mmHg) to {186.4}±{15.3}/{93.1}±{11.2} (mmHg) after 10 minutes (~*P {0.05}) and {158.3}±{14.7}/{84.3}±{8.3} (mmHg) after 60 minutes respectively (~{**}P {0.001}). At the same time HR and ECG didn’t change obviously ({88.6}±{18.6} bpm vs {81.4}±{14.9} bpm,P{0.05}). Conclusion The data indicate that intravenous diltiazem infusion is effective and safe for perioperative patients with hypertension.
