Objective We retrospectively reviewed our series of 18 consecutive patients who underwent odontoid screw fixation,to further define the safety and efficacy of the technique. Methods From 1998 to 2004, 18 patients presented with acute traumatic TypeⅡodontoid fractures were treated by odontoid screw fixation, 3 female and 15 male were included with average age of 42.2 years. All patients underwent the operation within 5 days after injury and rigid cervical collar were used for 6 weeks postoperatively, and serial clinical and radiographic examinations were monitored during the following-up. Results With a mean follow-rp period of 15.8 months, radiographic fusion was documented for 17 of 18 patients. No complications related to the surgical approach or hardware failure were identified, and all patients remained in neurologically stable condition. Conclusion Single-screw anterior odontoid fixation was associated with a relatively low complication rate and a high fusion rate in this study. We think that it is an effective treatment method for acute TypeⅡodontoid fractures.
