Influence of Mood Disturbance on Metabolic Control and Compliance with Treatment in Type 1 Diabetic Children

Objective To explore the influence of mood disturbance on the metabolic control and compliance with treatment in type 1 diabetic children.Methods The mood state of 90 children with type 1 diabetes was assessed by using the screen for child anxiety related emotional disorders (SCARED)and depression self-rating scale for children (DSRSC).The subgroup of mood disturbance included children whose score was equal or higher than the cut point of either SCARED or DSRSC,and the other children who had no mood disturbance were control group.The results of metabolic control evaluated by hemoglobin Alc (HbAlc) and compliance with treatment including home-glucose tests,diet modification and exercises were compared.Results 1. The mean value of HbAlc level of mood disturbance group was significantly higher than that of control group(8.82%±2.10% vs 7.79%±1.53%,t=2.496 P0.05),and the ratio of lower HbAlc level (≤7.0%) in the mood disturbance group was significantly lower than that of control group(26.5% vs 41.1%,P0.01).2. Children in mood disturbance group were significantly less likely to be compliant with the treatment regarding home-glucose tests,diet modification and exercises(Pa0.01).Conclusion Mood disturbance has significant effect on metabolic control and the compliance with treatment in type 1 diabetic children,and those with mood disturbance has poor outcome in both metabolic control and the compliance with treatment.
