Electrophysiologic changes of radicular cervical vertebrae diseases treated with needle-scalpel treatment

AIM: To observe electrophysiologic changes before and after needle-scalpel treatment for radicular cervical vertebrae diseases and investigate the feasibility and effectiveness of needle-scalpel treatment for radicular cervical vertebrae diseases.METHODS: ① Ten cases (5 males and 5 females, aged from 41 to 63, all knowing the treatment plan and testing items) with radicular cervical vertebrae diseases were selected from Pain Department of Fuzhou General Hospital of Nanjing Military Area Command of Chinese PLA from August 2002 to March 2004. ② Using tiny needle and scalpel Type Ⅳ or Ⅲ, conforming to the four-part needle entered principle of needle-scalpel treatment, scalpel edge was away from nerve and blood vessel, and the needle and scalpel entered above the cervici-skin, separating, with the needle and scalpel, interspinal ligaments and relevant muscle, ligaments fascia and joint capsule. Firstly, cutting vertically, and then peelling off horizontally if the tubercle needed cutting apart. When the needle was pulled out, pressed the pinhole for a while to avoid bleeding, and then observed for 20 minutes. If there was acupuncture syncope, massage therapy was necessary with once every five days, three times for a period of treatment, and three periods were required. ③ Examining patients' conduction velocity, latent period and induced potential amplitude of median nerve (sensory/motor), ulnar nerve (sensory/motor), radial nerve (sensory/motor), musculocutaneous nerve (motor), axillary nerve (motor) with the help of ESAOTE produced by Italian Baisheng Company. ④ Diversity comparison of the measurement data was examined with t test.RESULTS: All the ten patients were involved in the result analysis. ① After the needle-scalpel treatment, the electrophysiological exam showed the nerves of 50 pieces of muscle were damaged before the treatment (100 pieces of muscle among the 10 patients were tested, 50 pieces respectively before and after the treatment) and restored with different degrees after the treatment. ② The latent period (motor) of median, ulnar, radial, musculocutaneous, and axillary nerve was shorter than that before treatment (P 0.05) the induced potential amplitude (motor) of median, ulnar, radial, musculocutaneous, and axillary nerve was higher than that before treatment (P 0.05-0.01) the conduction velocity of median, ulnar, radial nerve was faster than that before treatment (P 0.05-0.01). ③ The latent period (sensory) of median, ulnar, radial, musculocutaneous, and axillary nerve was shorter than that before treatment (P 0.05-0.01) the induced potential amplitude (motor) and conduction velocity (sensory) of median, ulnar, radial, musculocutaneous, and axillary nerve were apparently higher or faster than thoset before treatment (P 0.05-0.01).CONCLUSION: Needle-scalpel therapy can improve the electrophysiological testing results of the patients with radicular cervical vertebrae diseases. Therefore, it is a quite feasible and effective approach.
