Comparison of related factors of anxiety and depression in patients with different diseases in the internal medical department of polyclinic

AIM:To study the difference of anxiety and depression in inpatients and outpatients in polyclinic with different sexes,ages,educated degrees and medical departments.METHODS:100 patients from different medical departments of some polyclinic from September 25th to October 14th 2003 were selected,including 63 cases of inpatients,37 cases of outpatients,and 25 cases respectively from 4 departments.They were assessed with the Hospital Anxiety and Depression Scale,which composed of 2 subscales and 14 items.In the anxiety subscale,there were 7 items focused on psychiatric symptoms of neurosis;and in the depression subscale,there were 7 items reflected lacking of pleasant,mental delay and depression.There were 6 reverse questions,5 of which in the depression subscale and 1 in the anxiety subscale.Every item was evaluated by the 0-3 grading,total score of each item was 21 scores.Nine scores recommended by that scale were regarded as the critical value of anxiety or depression,those with scores 9 had anxiety or depression, 9 had no anxiety or depression.The self-designed General Condition Questionnaire was used to evaluate the demographic data,and the content was as follows:①sex;②age(3 groups were divided according to age: 40 years,41-60 and 60);③educational degree(3 groups were divided according to preliminary school or below,middle school and vocational school,advanced vocational institutions or above).The different departments and clinical diagnosis were selected as the clinical data.With the unified guide,the patients were self-evaluated according to the real condition without pressure.The questionnaires were retrieved after checking.RESULTS:100 scales were handed out and all were collected with the efficiency rate of 100%.①In the 100 patients,there were 15 ones with pure anxiety,12 ones with pure depression,and 13 ones with anxiety with depression.The proportion of anxiety and depression were 28% and 25%,respectively.②No difference in anxiety and depression among patients with different sexes,ages and medical departments were found,while there were significant differences in patients with different educational degrees:the lower the education,the more the patients with depression,the occurrence rate of depression in patients with preliminary schools or below was 47.1%,the rates of patients with middle schools and advanced vocational institutions or above were 26.8% and 7.4%,respectively.CONCLUSION:The anxiety and depression of patients with different diseases in polyclinic are related to their education degree,the lower the education,the higher the occurrence rates;but have no correlation to sex,age or department.With the comparison of anxiety and depression of patients in polyclinic,we obtain the evaluated results of clinical psychology,which provide data for consultation-liaison psychiatry and treatment and nursing of psychophysiological disorder in internal medicine.
