Comparison of curative effect of unilateral and bilateral anterograde cerebral perfusion in aortic arch surgery:meta analysis

Objective It is controversial that whether bilateral superior than unilateral cerebral perfusion.This systemic and meta-analysis study aimed to provide evidence for clinic by comparing these two technique.Methods PubMed, Embase and the Cochrane Library, CNKI and Wanfang data base(from their inception to Jan 2017) were searched for cohort studies reporting on postoperative mortality and permanent(PND) and temporary neurologic dysfunction(TND) in aortic arch surgery requiring circulatory arrest with bilateral or unilateral antegrade cerebral perfusion. Screening literature, extracting date and assessing the risk biase of including studies was independently accomplished by two researches. Then RevMan 5.3 software was used to perform meta-analysis. Results 9 studies were analyzed for a total of 1411 patients receiving UACP versus 1480 receiving BACP. Pooled analysis showed that there were no statistical significant between the two groups in postoperative mortality(RR =0.88,95%CI0.71-1.08,P=0.22). TND(RR = 1.00,95%CI 0.81-1.24,P=0.99) and PND(RR= 1.00,95%CI0.81-1.24,P=0.99). Conclusion There was no significant difference in postoperative mortality and neurological dysfunction between unilateral and bilateral antegrade cerebral perfusion in the aortic arch surgery.
