Glucocorticoid-based combined with antibiotics therapy in the treatment of sudden deafness with elevated leukocyte counts

Objective To study the relationship between the level of leukocyte and the curative effect of the conventional treatment,which mainly consists of glucocorticoid,vasodilators and vitamin in sudden sensorineural hearing loss( SSNHL) patients. To investigate whether combined with antibiotics can improve the clinical efficacy of SSNHL with elevated leukocyte counts and to study the pathogenesis of sudden deafness with elevated leukocyte counts. Methods A total of 102 patients diagnosed with SSNHL were included in this study. Patients were divided into two groups according to leukocyte counts level. Traditional glucocorticoid based therapy was administered in normal leukocyte counts group( 61 patients). Patients with elevated leukocyte counts were then randomly divided into another two groups. 19 patients were given traditional glucocorticoid based therapy,and 22 patients were given glucocorticoid based combined with antibiotics therapy. Pure tone audiogram was performed before and after the therapy to evaluate the effects. Results The total effective rate of the conventional treatment was 62. 5%,the normal leukocyte counts group was 68. 9%,while the elevated leukocyte counts group was 42. 1%. The normal leukocyte counts group had statistically significant hearing improvement compared with the elevated leukocyte counts group( P 0. 05). The effective rate of the combination therapy was 77. 4%,while the conventional treatment alone was 42. 1% in the elevated leukocyte counts group. Combination therapy had statistically significant hearing improvement compared with conventional treatment alone( P 0. 05). Conclusion Bacterial infection may be associated with the pathogenesis of SSNHL with elevated leukocyte counts. The level of leukocyte can provide evidence for selection of clinical treatment for SSNHL.
