Clinical features of cerebral gliomas:analysis of 10 cases

Objective To investigate the clinical features,radiological and pathological features of gliomatosis cerebri,in order to improve the diagnosis and treatment.Methods From July 2007 to October 2013,a retrospective analysis of the clinical data of admitted pathological diagnosed brain glioma patients(10 eases) was performed.Results In the 10 patients,there were 6 males and 4 females,aged from 28 to 75 years old,median 47.4 years of age,combined with increased intracranial pressure,neurological deficits or irritating symptoms onset,skull imaging examination showed diffuse lesions,involving ≥3 cerebral lobes.Cranial CT scan showed low density,head MR scan showed slightly lower signal on T1W1,T2W1and FLAIR showed high signal,enhancement scan showed no enhancement or only mild enhancement.All 10 patients were pathologically confirmed,five cases in line with WHO Ⅱ- grade Ⅲ,4 patients in grade Ⅱ,1 case of grade Ⅲ.Pathological examination showed abnormal proliferation of glial cells with mild nuclear shaped,seven cases with diffuse astrocyte-based,two cases with diffuse mainly oligodendrocytes,one case was astrocytes hyperplasia concomitant oligodendrocyte proliferation,revealed "structural growth".Immunohistochemical staining showed GFAP(+ or + +),in some cases Olig-2(- or +) P53showed between 5%- 75%(+) except the main types are oligodendrocyte proliferation,Ki 67 in 5%to 50%(+).Six cases underwent surgery,one case was lost of follow up,rest five cases were survived at the end of follow up;in the 4 cases of non-surgical patients,2 patients died,survived of two cases.Conclusion Gliomatosis cerebri has low incidence.The clinical manifestations are not specific,imaging and pathological examination showed characteristic changes,combined with imaging and pathological findings can improve the accuracy of diagnosis,operation excision of the lesion may prolong the survival time than non-operation treatment.
