RELATIONSHIP BETWEEN THE DISTRIBUTION OF KESHAN DISEASE AND SELENIUM STATUS

The distribution of selenium in hair, blood and corn in both affected and nonaffected areas of Keshan Disease in China was determined. It was found that selenium concentration of individual blood samples was exclusively below 0.02 ppm from affected areas and there was an overlap (within the range of 0.02-0.05 ppm) between values from affected and non-affected areas, but the number of samples from affected areas becomes fewer as concentration of selenium becomes higher within this range. The averages (ppm±SD) of population blood selenium contents in affected and nonaffected areas were 0.021±0.001 and 0.095±0.088 respectively without overlap. Most of the population hair contents were below 0.12 ppm in affected areas but it mounted above 0.2 ppm in nonaffected areas. Those from nonaffected areas situated near the affected areas fell in between. The population hair contents averaged 0.07 and 0.343 ppm in affected and nonaffected areas respectively. Most of the selenium contents of staple corn and rice produced in affected areas were below 0.01 and 0.02 ppm respectively. Average of the former was 0.005 ppm and of the latter was 0.007 ppm.In order to ascertain the possibility that selenium insufficiency could be responsible for its prevalence, it was necessary to examine whether this insufficiency was in coincidence with all the epidemiological characteristics of this disease. Based upon the selenium contents of hair and staple cereal samples taken according to the epidemiological characteristics of Keshan Disease, it revealed that those from endemic areas were exclusively with lower selenium levels; hair selenium contents of susceptible populations were always higher than its counterparts. No further decrease in hair selenium contents was found during heavy prevalence season.It was suggested that lack of available selenium in the soil of affected areas caused lower selenium content ia staple cereals and in tura blood and hair selenium contents. This would be the "soil factor'', responsible for the occurrence of Keshan Disease, but other factors including biological agent might also play certain role.
