Clinical application and therapeutic effectiveness of large diameter heads metal-on-metal total hip replacement

Objective To explore the key operation techniques,postoperative early rehabilitation,indications and short-term results of total hip replacement(THR) using large diameter heads(LDH) in metal-on-metal(MOM) articulation. Methods A consecutive series of 16 patients(19 hips) who underwent THR using metasul LDH in MOM articulation from March 2006 to August 2008 were studied.There were 10 men and 6 women with a mean age of 46 years(range, 30-69 years).There were 7 left and 12 right hips arthroplastics performed.Using the poster-lateral approach,THR was performed with minimum thickness acetabular cup and LDH(range,48-56 mm).Results No intraoperative complications such as intraoperative fracture,nerve palsy,or vascular injury was observed.Postoperatively there was no dislocation or deep vein thrombosis.In all patients,leg length discrepancy was within 5 mm difference.The average Harris hip score at the last follow-up were significantly improved compared with the preoperative scores(90.7 points versus 43.6 points).The average range of motion(ROM) of hips at the last follow-up was significantly improved compared with the preoperative values. Conclusion The short-term results of LDH in MOM total hip replacement on ANFH,osteoarthritis,rheumatoid arthritis,femur neck fracture,dysplastic hips,ankylosing arthritis are excellent.
