Evaluation of ventricular synchrony and systolic function in patients with right bundle branch block by tissue synchronization imaging

Objective To investigate the inter- and intra-ventricular synchronization in the subjects with right bundle branch block (RBBB) by tissue synchronization imaging(TSI). Methods GE Vivid 7 ultrasound system with TSI and Q-analyze software was used. Twenty-four subjects whose electrocardiograms were shown as RBBB were enrolled in the study, and 25 normal subjects were included as the control group. Results ①About two-thirds subjects with RBBB had a delayed and higher isovolumic contraction (IVC) wave in the right ventricular(RV) free wall. ②After corrected by the heart rate, the time to peak velocity(T_C) of RV free wall in RBBB group was longer than that of control group. ③The stand deviation of T_C(T_ SD ) of all RV segments of RBBB group was larger than that of control group( 45.34 ± 18.63 ) ms vs ( 39.97 ± 11.17 )ms,P= 0.037 . ④The Tc difference between the latest RV segment and the earliest left ventricular segment in RBBB group was longer than that of control group( 118.29 ± 38.25 )ms vs ( 91.43 ± 48.54 )ms,P= 0.04 . Conclusions There is inter- and intra-ventricular asynchrony in patients with RBBB by TSI. T_C and T_ SD may be reliable parameters for evaluating RV synchrony.
