Expression and significance of apoptosis and proliferation of human hepato- cellular carcinoma

Objective To investigate the morphology and distribution of the apoptotic cells,and the relationship between apoptosis,proliferation and the histologic grading of human hepatocellular carcinoma(HCC).Method 21 cases of human HCC and 17 cases of chronic liver disease were analyzed by labeling the apoptotic cells with TUNEL technique and PCNA by immunohistochemical method.Results(1)There was a positive correlation between the TUNEL labeling index(TUNEL LI)and nuclear condensation,concentrated chromatin and eosinophi- lic cytoplasm;(2)Most of the apoptotic cells were scattered in distribution or located around the necrotic tumor area,but being less in tumor infiltrating area;(3)The TUNEL LI of grade Ⅲ,Ⅱ and Ⅰ of HCC were 2.1±0.97,0.92±0.26 and 0.45±0.08,and the PCNAL LI were 27.8±6.4,14.1±3.5,7.42±0.8,respectively.Both TUNEL LI and PCNAL LI were positively correlated with histologic grading of the HCC,P0.05~ 0.001);(4)The TUNEL LI and PCNA LI of the capsule infiltrated group and portal vein tumor thrombus group were higher than those in the noninfiltrated and absence of tumor thrombus groups.Conclusion This study shows that the apoptotic LI and PCNA LI are highly positive and correlated with the histologic grading,capsuler infiltration and portal vein tumor thrombus.Apoptosis may thus play an important role in the growth pattern of human HCC.
