Surgical treatment thoracoabdominal aortic aneurysmswith hlocking of aorta at short Intervals

AbstractVisceral ischemia and deterioration in renal func-tion is cornmon cause of morbidity in patients withthrocoabdominal aortic aneurysm surgically treated.Two patients with hiige thoracoElbdoniinal aortic aneurysm were surgically treated by a new technique ofshunting. The operation was performed by using asynthetic artificial graft,which was sutured to the iliacarterybyEndto smeanastomosis,and the upper end of the graft was anastomosed to the cut end of the tho-racic aorta by end-to-end rnethod.Compared with thetraditional temporary shunting,there are several ad-vantages in using the improved shunting technique.First,by so doing the ischeniic time of various visceracould be fractionated and shortend,just about 17 min-utes. visceral ischemic complication can be avoided.Second the pseudoaneurysm produced by the tradition-al method of temporal shunting could be prevented bythe improved method. Third, on account of the is-chemic time of the viscera is decreased,operation couldbe done under normal temperature without need forhypothermia. In treating a case of huge throcoabdomi-nal aortic aneurysm,successful result could hardly beobtained without good prepostoperative care,Weshould also emphasize that the improvement of somesurgical technique is also important
