The Study of the clinical features and risk factors of postoperative delirium in patients with vascular surgery

Objective To study the clinical features and risk factors of postoperative delirium (POD) in vascular surgery patients.Methods The clinical records of 54 patients suffering from postoperative delirium in vascular surgery were collected and analyzed retrospectively.Results average age was 72.80±9.18 years old.Thirty-three patients (61.11%) had abdominal aortic aneurysm.Among them,28 patients (84.84%) underwent the artificial blood vessel shunting for the ilial/femoral arery stenosis or occlusion.The plasma albumia and hemoglobin in POD group were lower compared with the control group (P0.01).The incidences of the hypertension (77.78%),coronary heart disease (CAD,55.56%),cerebral vessel disease (CVD,77.80%),postoperative pyrexia (55.56%) were higher compared with the control group(P0.01).The incidences of diabetes (33.33%) and renal dysfunction (33.33%) were also higher compared with the control group(P0.05).The minimum systolic blood pressure in the POD group was 111.11±19.73 mmHg which was lower compared with the control group(P0.01).The duration of the minimum blood pressure in the POD group was 83.89±34.83 min which was longer compared with the control group(P0.01).Logistic regression analysis showed that CVD,postoperative pyrexia,minimum blood pressure and the long duration of the minimum blood pressure during vascular surgery were the risk factors of POD.Conclusions The postoperative delirium in vascular surgery patients tend to occur in elderly patients undergoing surgery operation of abdominal aortic aneurysm and ilial/femoral arery stenosis or occlusion with artificial blood vessel shunting.The risk factors of POD include CVD,postoperative pyrexia,and the long duration of the low blood pressure during vascular surgery.
