Retrospective review on two cases of transfusion related acute lung injury

Objective To analyze the risk factors in two cases of blood transfusion-related acute lung injury( TRALI),and to provide reference for the prevention of TRALI. Methods The clinical data on the two cases of TRALI were retrospectively analyzed. The pathogenesis,diagnosis and treatment of TRALI were reviewed. Results The two cases of TRALI patients both were infused with transfused blood containing high amount of plasma during traumatic surgery,of which,one patient was cured,the other patient died. The blood infused in the two cases of TRALI patients came from four blood donor:two were multipara female blood donors,other two were male blood donors. A follow-up on the past recipients from these four blood donor has found no cases of TRALI reaction. Conclusion Blood transfusions carry major complications,including trauma,which confers the major risk in developing TRALI. Blood products that are rich in plasma constitute the direct cause of TRALI in blood transfusion patients. Patients with high risk factors in developing TRALI should minimize infusion in order to prevent the occurrence of TRALI.
