Objective To explore the moderating effect of cognition in blood donation between the act of blood donation and health-related quality of life. Methods 839 participants who had experience in donating whole blood were recruited into the cross-sectional survey. The MOS 36-item short-form health survey(SF-36) and the cognition in voluntary blood donation questionnaire were used to evaluate health-related quality of life(HRQOL) and the level of cognition in blood donation respectively. The act of blood donation was assessed by cumulative frequencies of blood donation. The moderating effect of cognition in blood donation between the act of blood donation and health-related quality of life was analyzed by multiple regression analysis. Results The results revealed that the act of blood donation had a positive influence on quality of life both in physical field(PCS)(b′=0.111,P0.05) and mental field(MCS)(b′=0.099,P0.05). The moderating effect test showed that the effect of cognition in blood donation between the act of blood donation and PCS was significant(Interaction item(b′=-0.068,P0.05). The coefficient of the interaction was negative, which means if donors were at a level of high cognition, the cumulative frequencies of blood donation had a smaller positive impact on physiologic condition than at a low level. There was no significant difference in the moderating effect of cognition in blood donation between the act of blood donation and MCS(Interaction item b′=-0.057,P0.05).Conclusion Under the regulation of cognition in blood donation, health-related quality of life of donors will raise with the increasing of cumulative frequencies of blood donation. When repeated donors unconsciously cultivate a habit of blooding donation, the effect of cognition in blood donation between the act of blood donation behavior and health will be weakened.
