Applied Anatomy Study of Sphenopalatine Artery

Objective: To explore the factors affecting the sphenopalatine artery ligation operation through the observation of sphenopalatine artery and the relevant anatomical structures to provide theoretical guidance for reducing the bleeding during the operation,obtaining satisfactory operation view,selecting proper surgical management and avoiding operation complications. Methods: 30 cases of dry half skull specimens were taken to observe the location of the sphenopalatine foramen and to measure the distance between anterior ethmoidal foramen and anterior nasal spine,the vertical distance between the ethmoidal crest and the horizontal plate of inferior turbinate and the vertical distance between the ethmoidal crest and the horizontal plate of palatine bone. The detailed dissection of lateral wall of nasal cavity,maxillary sinus and pterygopalatine fossa was performed under the operation microscope in 15 adult cadaveric heads( 30 sides) with no abnormal appearance and fixed by 10 % formalin( without distinction of sex) to observe sphenopalatine artery and its relevant anatomical structures. Results: The crista ethmoidalis and sphenopalatine foramen were important signs for locating sphenopalatine artery by nasal cavity. The maxillary artery runs in the pterygopalatine fossa and falls into three branch types( type Y,intermediate type and type M),with individual differences among the branch types. The ligation of the sphenopalatine artery at the posterior wall of the maxillary sinus could not cause damage to the mandibular segment of the maxillary artery and the pterygoid branches. Conclusion: The relevant anatomy study of sphenopalatine artery is of great significance for the sphenopalatine artery ligation and the familiarity with the anatomy structures relevant to the location of sphenopalatine artery should be achieved before operation. The sphenopalatine artery ligatione at middle meatus of the posterior wall of the maxillary sinus is a method for choice and the branch ligation of the sphenopalatine artery can be performed through the nasal cavity. The pterygopalatine segment of the internal maxillary artery varies greatly in pterygopalatine fossa,to which much attention should be paid to avoid damage to cause complications.
