Study on the morphological changes and the clinical values of total lumbar vertebral body and inervertebral disc in the lumbar degenerative spondylolisthesis

Objective To explore the clinical values of the morphology changes of total lumbar vertebral body and inervertebral disc in the lumbar degenerative spondylolisthesis.Methods Patients with lumbar degenerative spondylolisthesis(L4)diagnosed by CT from January 2011-January 2016 in Yantaishan hospital were selcected as the case group.Sixty patients were randomly selected:male 30 cases,female 30 cases,aged 51-89 years old,averaged 67.9±7.5years old.Sixty patients without lumbar degenerative spondylolisthesis were selected as control group with gender and age matching,male 30 cases,female 30 cases,aged 45-85 years old,averaged 64.6±9.6years old.On the median sagittal position image,measurements were carried out as follows:vertebral body height(VBH,anterior/middle/posterior),intervertebral disc height(IVDH,anterior/middle/posterior),lumbar lordosis angle(LLA),intervertebral disc angle(IVDA).On the transverse position image,measurements were carried out as follows:vertebral body length(VBL,superior/inferior),vertebral body width(VBW,superior/inferior).With the single factor analysis of variance,we compared the case group and control group differences between the measurements.With the Logistic regression analysis,we analyzed the association of angle of the lumbar vertebral body and intervertebral disc between the two groups.Results Compared with the control group,the changes of women's vertebral body:L1 middle VBH,L3 anterior and middle VBH became shorter,L5 inferior VBL became longer,L2 superior and inferior VBW and L3 superior and inferior VBW became wider,and L5 LLA became smaller.The changes of women's intervertebral disc:L1/2 posterior IVDH,L2/3posterior IVDH,L3/4posterior IVDH,L4/5anterior and middle IVDH,L5/S1 anterior and middle IVDH became shorter,L1/2IVDA and L3/4IVDA became greater,L5/S1 IVDA became smaller.Compared with control group,the changes of men's vertebral body:L3 middle VBH became shorter;L2superior and inferior VBW and L3 superior and inferior VBW became wider,L1 LLA became smaller,and L4 LLA became greater.The changes of men's intervertebral disc:L3/4 middle and posterior IVDH,L4/5anterior,middle and posterior IVDH,L5/S1 anterior IVDH became shorter,and L2/3IVDA,L4/5IVDA became smaller.Logistic regression analysis showed that LDS was significantly related to L4 LLA,L2/3IVDA,L3/4IVDA,and L5/S1 IVDA,and the regression coefficients were 0.718,1.330,0.900,and1.093 respectively.Conclusion There are some certain correlations between the changes of L4 LLA,L2/3IVDA,L3/4IVDA,L5/S1 IVDA and LDS.
