Clinicopathological significance of the expression of ACE and the AT1 receptor in muscular dystrophy

Objective To explore the contributive role of the rennin-angiotensin system (RAS)in muscular dystrophy(MD).Methods The expression and cellular localization of angiotensin-converting enzyme(ACE)and the angiotensinⅡtype 1(AT1)receptor in biopsied muscle specimens from patients with Duchenne muscular dystrophy(DMD),Becker muscular dystrophy(BMD) and congenital muscular dystrophy(CMD)were detected with immunohistocbemistry,double immunofluorescence labeling and Western blot assay.Results Immunohistochemistry and double immunofluorescence labeling found that ACE was positive for vascular endothelial cells of the normal muscle,but the AT1 receptor was expressed predominantly in vascular smooth muscle cells of the normal muscle,Immunohistochemistry and Western blot analysis showed that immune reaction of ACE and the AT1 receptor were intensified significantly in dystrophic muscles of patients with MD,which were distinctly expressed in the membrane,cytoplasm and nucleus of regenerated muscle fibers,fibroblasts, macrophages and macrophage infiltrating necrotic fibers.Double immunofluorescence labeling showed that activated fibroblasts in endomysium and perimysium of DMD and CMD muscles were positive for ACE and the AT1 receptor,especially eminently for the AT1 receptor.Immune reaction of the AT1 receptor was in parallel with the increased activity of ACE.Conclusion The local increase in the expression of ACE and the AT1 receptor indicates that the RAS of muscular tissues has been activated during muscular dystrophy,which might contribute to the pathogenesis of MD.
