Application of auxiliary small incision in internal fixation of unstable intertrochanteric fracture

Objective To investigate the effect of assisted small incision in the treatment of unstable intertrochanteric fractures with closed reduction and interlocking intramedullary nail(PFNA).Methods A total of 35 cases of unstable intertrochanteric fractures in our hospital from 2014 to2016 were selected.The posterior superior inferior tibiofibular incision was combined with the posterior incision of the femoral trochanter and the periosteum vibrator was used to apply a certain stress to the fracture end.An anatomical or near-anatomical reduction of the fracture end was obtained,and the PFNA was fixed after satisfactory reduction.The patients were followed up for 1 to 2 years.The Salvati-Wilson hip function score and X-ray diagnosis were used to evaluate the fracture healing.Results After the operation,the patients were examined by X-ray.The X-ray showed good fracture healing.There was no looseness or fracture in the internal fixation.The Salvati-Wilson hip function score was excellent in 18 cases,good in 14 cases,and fair in 3 cases.The excellent and good rate was 91.4%.Conclusion The anterior superior iliac spine assisted incision combined with the oblique posterior incision of the femoral trochanter is more favorable for the anatomical or near anatomical reduction of the unstable intertrochanteric fracture.The clinical effect is exact.
