Comparison of urodynamic characteristics between premenopausal and postmenopausal stress urinary incontinence

Objective To study the difference of urodynamic characteristics between premenopausal and postmenopausal stress urinary incontinence (SUI) and try to find out the different pathogenesis. Methods A retrospective study was done in 116 adult female SUI patients (66 premenopausal cases and 50 postmenopausal cases) who underwent abdominal leak point pressure (ALPP) and urethral pressure profiles (UPP). The urodynamic characteristics of each patient were analyzed and the results were compared between the two groups. Results ALPP of premenopausal group was much higher than that of postmenopausal group [(80±28) cmH 2O vs (65±28) cmH 2O, P0.05]. The patients whose ALPP under 60 cmH 2O in premenopausal group was 14, less than 19 in postmenopausal group (21.2% vs 38%, P0.05), whereas the patients whose ALPP above 90 cmH 2O were 20 in premenopausal group, more than 7 in postmenopausal group (30.3% vs 14%, P0.05). While MUCP and continence area showed significant difference (P0.05), functional urethral length showed no statistic difference between the two groups. Conclusion Premenopausal and postmenopausal SUI showed different urodynamic characteristics. It implies that the pathogenesis of SUI in the two groups was dissimilar.
