A survey on the iodine nutritional status and thyroid function of pregnant and lactating women in Jilin province

Objective To understand the iodine nutritional level and thyroid function of pregnant and lactating women in Jilin province,in order to provide the scientific basis for policy making in iodine deficiency disorders(IDD) control.Methods 30 counties were selected from Jilin province by PPS sampling method,1 primary school was selected by simple random sampling method in each selected county.In each school,40 children aged 8~10 were randomly selected for thyroid examination,the household edible salts tested with quantitative method in laboratory,and the urinary samples were tested from 12 of the 40 children.In the village(community) where the selected school were located,1 drinking water sample was collected according to their positions of east,south,west,north and center,2 drinking water samples were collected to test water iodine in the centralized water supply areas.3 townships(sub-district) were selected,8 pregnant and lactating women were selected in each township(sub-district).Total 24 pregnant and lactating women in each selected county were tested urinary iodine and thyroid function.Urinary iodine were determined by As-Ce catalysis spectrophotometry method,the household edible salts were tested by direct titration method,and thyroid function were measured with chemiluminescence,thyroid volume was examined in 8-10 years old children by ultrasonography.Results Totally 1 242 aged 8~10 children were surveyed,and the median UI were 195.30 μg/L,the prevalence of goiter evaluated by B ultrasonography was 1.84%,the median iodine salt was 31.3 mg/kg,the median iodine content in drinking water was 5.81 μg/L.The median UI of pregnant and lactating women were 185.65 μg/L,173.56 μg/L,respectively.The rates of the overall level of thyroid dysfunction were low in pregnant and lactating women,among them mostly were subclinical hyperthyroidism(2.9%),hypothyroxinaemia(2.8%),subclinical hypothyroidism(2.3%);The rates of the three types of thyroid dysfunction were high in lactating women and less hyperthyroidism;the rates of thyroid dysfunction increased significantly in women with antibody positivity,among them mostly were subclinical hypothyroidism(9.2%),subclinical hyperthyroidism(4.6%) and hyperthyroidism(4.0%).Conclusions The iodine nutritional level of pregnant and lactating women was appropriate,the current salt iodine concentration(30 mg/kg)was optimal for them.We should highly concern about the status of iodine nutrition of pregnant and lactating women,and put them into routine monitoring.Early screening is necessary for subclinical thyroid dysfunction in pregnancy(or women desiring pregnancy).
