Preliminary Evaluation of Thoracolumbar Spine Injury Classification and Severity Score System

Objective To evaluate the interobserver reliability and intraobserver reproducibility of the thoracolumbar spine injury classification and severity score(TLICS) and identify the clinical decision-making in the treatment of thoracolumbar spine injury.Methods Forty-eight consecutive patients with thoracolumbar vertebra fracture were enrolled in this study.Each of these patients underwent a thorough history and serial physical examinations followed by a comprehensive radiographic evaluation consisting of plain radiographs,computed tomography scanning,and magnetic resonance imaging.These injuries were consecutively scored,using the TLICS.Three months later,the clinical and radiographic data for each of the cases in this series were randomly reordered and reevaluated with the same algorithm.Cohen's unweighted kappa coefficients were calculated for the key parameters of each classification system to analyze the reliability and reproducibility of TLICS.Results The interrater kappa statistics of all subgroups were within the range of moderate to substantial reliability(0.43~0.72).The interrater kappa statistics of all subgroups were within the range of moderate to substantial reliability(0.55~0.76).The rates of diagnosis correct,sensitivity,specificity were 95.1%,87.5% and 96.8% respectively.Conclusion The TLICS has good reliability and reproducibility designed to simplify injury classification,evaluate exactly and facilitate treatment decision making of thoracolumbar spinal column injures.
