Toward the Horizontal Equity:The Inequality of Medicaid's Benefit Level in Rural China

Recently, with the real payment level of China's Rural Medicaid scheme growing significantly,the problem of inequality of benefit level is becoming increasingly serious. Based on prefecture city datafrom 2006 to 2012, this paper has found that although the level of Medicaid's per capita payment keptrising, there still exist serious inequality problems. The average Gini coefficient and Theil index of its ben-efit levels across regions were as high as 0.492 and 0.633 respectively. The inequality decompositionshowed that the regional disparity comes from within-regions' inequality other than between-regions', espe-cially in east area. Besides, the inequality degree of the "first assistance"(ensuring all the target groupsof Rural Medicaid to participate in New Rural Corporative Medical System freely) was lower than the"sec-ond assistance"(partially covering out-of-pocket payment for curing serious diseases) and improving the in-put of the"first assistance" by incorporating more poverty groups into the basic medical security systemhad an obvious reducing effect on inequality.
