The effects of distraction of intervertebral body in treatment of Ⅱ° lumbar isthmic spondylolisthesis

Objective To observe the results of the Ⅱ°lumbar isthmic spondylolisthesis treated with podyrtion lumbar interbody tusion (PLIF)distraction between intervertebral bodys and supplemental instrumentation. Methods Thirty-seven patients with Ⅱ°lumbar isthmic spondylolisthesis underwent a PLIF(posterior lumbar interbody fusion), using single fusion cage inserted posterolaterally and oriented anterolaterally on the symptomatic side,and with same level underwent transpedicular screw rod fixation. Patients (28 to 52 years of age) had at least one year of low back pain and/or sciatica, and a severely restricted functional ability. The patients were followed up from 12 to 60 months. The stability and fusion of the disorder segments were observed on the X-ray film.Results All these 37 patients achieved successful radiographic fusion at 12 to 18 months. Clinical results: 23/37 patients were excellent, 14/37 were good, and no poor clinical results. No implant fractures or deformities occurred in all patients. Conclusion The aim of surgical treatment of isthmic spondylolisthesis is solid interbody fusion on the basis of adequate decompression of the neurological structures, and appropriate lumbar lordortic alignment and limited reduction through distraction of the interbody space using diagonal insertion of single threaded cage with supplemental transpedicular screw rod instrumentation.
