Dopamine Transporter PET Imaging in Patients with Progressive Supranuclear Palsy and Parkinson's Disease

Aim To investigate the distribution of striatal dopamine transporter in patients with Parkinson’s disease(PD)and progressive supranuclear palsy(PSP)by ~(11)C-CFT PET imaging,and to evaluate its value in the differential diagnosis of PD and PSP.Methods The clinical and ~(11)C-CFT PET imaging data of 40 patients with PD and 20 age-and course-matched patients with PSP were retrospectively analyzed,and another 20 age-matched healthy volunteers were selected as the control group.The ~(11)C-CFT uptake values of the three groups in caudate nucleus(C),anterior putamen(AP)and posterior putamen(PP)were calculated by region of interest(ROI)approach.Results PSP patients showed a faster disease progression and higher UPDRS score and Hochn-Yahr stage when compared with PD patients(P0.01).The ~(11)C-CFT uptake of PD and PSP groups in caudate,anterior and posterior putamen were significantly lower than that in normal control group(P0.01).The ~(11)C-CFT uptake of PSP group in each subregion was significantly lower than that in PD group(P0.001).And the ratio of DAT binding in anterior putamen to caudate(AP/C),posterior putamen to caudate(PP/C)and anterior putamen to posterior putamen was relatively higher in PSP patients than in PD patients(P0.05).The sensitivity of differential diagnosis of PD and PSP was 85%and specificity was 97.5%when using 1.20 as the cut-off value of the AP/C ratio,and the sensitivity and specificity of differentiating PD from PSP were 90%and 95%respectively,when using0.77 as the cut-off value of the PP/C ratio.Conclusion ~(11)C-CFT PET imaging can visualize the presynaptic dysfunction of dopaminergic neurons in PD and PSP patients,and the intersubregional ratios can effectively discriminate the two diseases.
