Quantitative Evaluation of the Post-systolic Shortening of Left Ventricular Wall in Patients with Myocardial Infarction by Tissue Doppler Imaging

Objective: Use velocity Doppler tissue imaging (DTI) to evaluate PSS and its difference in ratio and peak velocity among normal, acute and old myocardial infarction (MI) patients. Methods:42 patients were evenly divided into acute, old MI and control groups (group A, B, and C). GE vivid 7 was used to take 3 apical chamber images of each patient. Q-analyse software was used to analyze the Sm, PSS, Em, Am, Ds and εs on the 12 left ventricular (LV) segments. Biplane Simposon 's method was used to measure LVEF as comparison. Results: (1) After MI, the ratio of PSS increased, and the increase is more significant in Group A (P0.01). PSS Peak velocity also raised after MI, and still group A raised more(P 0.01). (2) MI patients showed severe impairment in Sm, Em, Am, Ds and εs comparing with the controls(P 0. 01), however there were hardly any difference between group A and B except for Em(P 0.01). (3) Mean PSS of the basal segments in the control group is positively related with LVEF(P0.05), while in MI patients,PSS velocity tend to be inversely associated with LVEF. Conclusion: PSS measured by DTI could be promising noninvasive modalities to evaluate subtle changes of LV segmental function.
