Palliative Treatment with Self-expandable Metallic Stent in Malignant obstruction of Inferior Vena Cava

Objective: To evaluate the effectiveness of endovascular deployment of metallic Z-type self-expandable stents in treating the patients with inferior vena cava (IVC) obstruction caused by hepatic malignant tumour. Methods: 126 patients with IVC obstruction due to malignant compression were enrolled into this study, including 102 cases of primary hepatic carcinoma and 24 cases of metastatic hepatic carcinoma. Before therapy, every patient underwent CT or MRI examination. All patients' clinical signs and symptoms due to malignant IVC syndrome were classified and scored. Venography was performed via right or left femoral vein before and after metallic Z-type self-expandable stent was implanted across the stenotic segment of IVC. The diameter of stenotic segment, collateral vessels, vein pressures and the scores of patients IVC syndrome were compared before and after stent placement. The patency of IVC was followed by CT or venography every two months. Results: 145 Stents were implanted in 126 patients successfully. The average obstructive length of IVC was 6.3±2.1cm. The pressure gradient of stenotic segment of IVC was declined from 2.1±0.5kPa to 0.5±0.12kPa. The diameter of stenotic segment of IVC was increased from 0.3±0.11cm to 1. 5±0.4cm. After operations, the main symptoms and physical signs relieved quickly in 1-3 days. There was no serious procedure-related complication except one patient experienced acute thrombosis in IVC on the second day after therapy. During 2-24 month follow-up, the patency of IVC stents reached 87. 2%. Conclusion: Endovascular deployment of metallic Z-type self-expandable stent was an effective palliative treatment for patients with malignant obstruction of IVC.
