Sequential Non-invasive following Short-term Invasive Mechanical Ventilation in the Patients Induced by COPD Sufferedtnion Ⅱ Respiratory Failure

Objective: To observe the efficacy of early extubation and sequential non-invasive mechanial ventilation(MV) in COPD with exacerbated hypercapnic Ⅱ respiratory failure. Methods; 28 intubated COPD cases with severe hypercapnic Ⅱ respiratory failure duo to pulmonary infection (pneumonia or purulent bronchitis) were involved in the study, with synchronized intermittent mondatory ventilation+pressure support ventilation(SIMV+PSV). At the time"Pulmonary infection control window" .extubation was conducted and followed by non-invasive MV via facial mask immediately, with PSV+PEEP(positive end-expiratory pressure). To observe the in fection,MV and SpO2. Results:28 cases the duration of invasive MV was 3~8 days, the total duration of ventilatory was 6-19 days,the incidence of VAP were 2. Conclusion:In COPD patients requieing intubation and MV for pulmonary infection and hypercapnic respiratory failure,early extubation followed by non-invasive MV may significantly decrease the invasive and total durations of ventilatory support,the risk of VAP ,and the duration of ICU stay.
