Effect of Acute Normovolemic Hemodilution with Polygeline on Renal Function

Objective: To investigate the changes in renal function induced by acute normovolemic hemodilution with polygeline injection . Methods: 90 patients scheduled for elective surgery were randomly divided into three groups; control group (Group D), Lactate Ringer's injection group (Group Dand polygeline injection group (Group J) The desired end-point HCT is 30% . After induction, blood was withdrawn from radial artery and collected in standard blood bags. And at the same time replaced with LR(3 times) or PG(1. 5 times). Vital signs were monitored and the amount of blood loss and transfusion were recorded . Hemoglobin, hematocrit platelet count, Blood coagulation and ACT was checked before and after blood hemodilution, BUN, Cr, blood and urea β2-microglobulins, Amount of urine , glomerular filtration rate and urea precipitation were checked before operation and 1 day and 7days after operation . Results: The amount of blood transfusion of foreign body was 450±23,200 and 0 ml in group D. L and J respectively. The MAP and CVP decreased and HR increased significantly 5 and 15 min in Group L after hemodilution (P0.05),there was no significant change in blood coagulation and Act in group J and L, the same was in amount of urine,glomerular filtration rate and urea precipitation in the three groups BUN Cr and β2-microglobulins were increased in all groups 1 day after operation and returned to pre-operation Level 7 days after operation but there were no significance between the there groups. Conclusion: Acute normovolemic hemodilution with polygeline injection has little effect on renal function and can significantly decrease the demand of blood products.
