Clinical Efficacy of Priming with Pipecuronium in Patients with Hepatic Failure

Objective: To investigate the priming dosage of pipecuronium on its relax onset and the intubation condition in patients with hepatic failure scheduled for liver transplantation. Methods: After anesthesia induction. 27 patients with Child-Pugh score of 7 to 9 were randomly divided into three groups(n=9, each). Thacheal intubation was facilitated with pipecuronium at total dosage of 3ED95. Patients in group 1,2,3 were primed with saline, 1/10 or 1/6 of total pipecuronium respectively 4 minutes before all pipecuronium injection. The onset time of pipecuronium in three group was recorded. Intubation condition and the safety were evaluated. Results: Onset time was 161s, 111s and 93s in group 1, 2, 3 respectively. There were significantly statistical differences between three groups. There was no difference of the intubation condition score between three groups. Value of T1/Tc in one patient in group 3 decreased to 85% and another to 92% 3 minutes after priming. No patient in three groups was found with decrease of SpO2,occurrence of diplopia or difficulty of swallow. Conclusion: Priming with 1/10 of 3ED95 dose of pipecuronium followed may accelerate the onset of the drug without affecting intubation condition and safety in patients with hepatic failure.
