Objective:To determine the efficacy of three dimensional-conformal radiotherapy combined with capecitabine chemotherapy for locally advanced or recurrent rectal cancer. Methods: Fifty-two patients with locally advanced or recurrent rectal cancer were irradiated by external beam radiotherapy to a total dose of 40 Gy. then randomized to receive 3DCRT plus chemotherapy (combined group) or 3DCRT alone (control group), with 20 patients in the former and 32 in the latter. The chemotherapy regimen is capecitabine 825mg/m2 daily in two divided doses days 1-14 every 3 weeks. Results; The pain-alleviation rates were 85% and 78.1 %,the overall response (CR+PR) rates were 90% and 78. 1 %, with complete response rates were 30% and 25% in the combined and control group. There were no treatment-related deaths and no grade 4 haematological toxicity. Conclusion: Three dimensional-conformal radiotherapy combined with capecitabine chemotherapy are effective in advanced or recurrent rectal cancer. It can result in down-staging of tumours. In addition, it is a well-tolerated regimen.
