THE DIAGNOSIS AND TREATMENT OF EARLY COLORECTAL CARCINOMA

Colonoscopy was performed in 5400 patients from 1984 to mid-1990 with detection of early colorectal carcinoma in 36 patients. Five of the 36 had in situ carcinomas and 31 submucous carcinomas. All these early cancers were malignant change of polyps and 74.9% were in the left colon. Polyps with erosion, bleeding and especially ulcer were usually malignant. In situ carcinomas were resected through colonoscope but malignant polyps over 2 cm in diameter were not suitable for colonscopic resection and should be resected through laparotomy. If the tumors resected through colonoscope were malignant villous polyps, sessile polyps, poorly differentiated or there were tumor cells at the cut margin, in lymphatic or blood vessel, radical operation should be done one or two weeks later.
